Laparoscopic Abdominal Trans-Anal Proctosigmoidectomy with Colo-Anal Anastomosis (LATA Resection)

Abstract
There have been many advances in the treatment of rectal cancer over the last few decades. Anal sphincter preservation
and total mesorectal excision have become essential elements of the treatment for the rectal cancer, especially in cases of low rectal cancer. In very limited situations where it is not possible to spare the safety margin and preserve the anal
sphincter, abdominoperineal resection is an alternative choice. Laparoscopic surgery not only minimizes the size of operative wound and reduces postoperative pain, but it also helps patients with early postoperative ambulation, and it provides surgeons an enhanced view of the operative field. Ever since applying distinctive laparoscopic anal sphincter-preserving technique for the low rectal cancer starting in 1996, the main author (JG Kim) has noted favorable outcomes with this operation. The surgery is called, ¡®laparoscopic abdominal trans-anal proctosigmoidectomy with colo-anal anastomosis¡¯, abbreviated as ¡®LATA resection¡¯. The background information pertaining to this surgery, the patient selection criteria, and the surgical techniques are presented in this paper. If oncologic results of this surgery turn out to be comparable or even superior to the conventional open procedure in the near future, LATA resection may become the preferred choice in the treatment of low rectal cancer, like other laparoscopic colon surgeries.
